hires if you are an external applicant. /n Position
Summary/Essential Duties: /n Organizes and conducts Physical
Therapy program to facilitate the rehabilitation of people with
physical and/or psychosocial disabilities. The Physical Therapist
directs the patient’s participation in selected
activities/treatments to restore, re-enforce and correct pathology
to promote and/or maintain good health. /n Essential duties
include: /n /n - Performs evaluation(s) and develops an effective
treatment plan appropriate to a patient’s rehabilitation needs
within the care continuum. /n - Performs effective treatment
techniques in a competent manner. /n - Educates
patient/family/caregiver in applicable health information,
treatment and prevention. /n - Initiates and participates in the
discharge planning process. /n - Performs other department
activities such as, but not limited to documentation, billing,
support of department operations, etc /n /n Required For All Jobs
/n /n - Performs other duties as assigned. /n - Complies with all
policies and standards. /n - For specific duties and
responsibilities, refer to documentation provided by the department
during orientation. /n - Must abide by all requirements to safely
and securely maintain Protected Health Information (PHI) for our
patients. Annual training, the UH Code of Conduct and UH policies
and procedures are in place to address appropriate use of PHI in
the workplace. /n /n This candidate will join a dynamic team. This
position will fill a weekend position to support weekend (and
holiday) rotation. The schedule will be determined during interview
process & mutually agreed upon prior to hire. There are three
options possible: Option 1: Week 1-SAT SUN, Week 2-SUN MON Option
2: Week 1-FRI SAT, Week 2-SAT SUN Option 3: Week 1 & 2-SAT SUN
Primary location will be Lake West Medical Center; opportunities to
float to other EAST Market UH facilities is possible. Therapist
with acute care experience preferred. Qualifications Education: /n
Graduate of an accredited Physical Therapy program /n /n Required
Credentials, License, and / or Certifications: /n Licensed by the
State of Ohio to practice Physical Therapy /n /n Experience &
Knowledge: /n Requires the evaluation and treatment skills
necessary to provide Physical Therapy appropriate to the
developmental and chronological age of patients on caseload /n /n
/n Special Skills & Equipment Knowledge: /n Professional
initiative, objectivity, good time management and interpersonal
skills. Must be physically, cognitively and perceptually able to
provide patient treatment and able to assist in physical mobility
of patients and equipment /n /n Physical Demands /n /n - Standing
