RN Field Case Manager plays a critical role in delivering
high-quality, patient-centered care within the home health setting
at Wooster Community Hospital. This position is responsible for
assessing, planning, implementing, and evaluating nursing care for
patients in their homes, ensuring that care plans are tailored to
individual needs and promote optimal health outcomes. The RN Field
Case Manager collaborates closely with interdisciplinary teams,
patients, and their families to coordinate services, manage complex
cases, and facilitate smooth transitions between care settings.
This role requires a proactive approach to patient education,
advocacy, and resource management to support patient independence
and prevent hospital readmissions. Ultimately, the position aims to
enhance patient quality of life through compassionate,
evidence-based nursing care delivered in the community environment.
Minimum Qualifications: Current and valid Registered Nurse (RN)
license in the state of Ohio. Minimum of two years of clinical
nursing experience, preferably in home health, case management, or
a related field. Basic Life Support (BLS) certification. Strong
knowledge of nursing principles, home health regulations, and
patient care standards. Ability to travel to patient homes within
the designated service area. Preferred Qualifications:
Certification in Case Management (CCM) or Home Health Nursing
(CHHN) is highly desirable. Experience with electronic medical
records (EMR) systems and documentation. Familiarity with Medicare
and Medicaid home health regulations and reimbursement processes.
Excellent communication and interpersonal skills to effectively
engage with diverse patient populations. Demonstrated ability to
work independently and manage a caseload efficiently.
Responsibilities: Conduct comprehensive patient assessments in the
home environment to determine nursing needs and develop
individualized care plans. Coordinate and manage patient care by
collaborating with physicians, therapists, social workers, and
other healthcare professionals. Monitor patient progress and adjust
care plans as necessary to achieve desired health outcomes and
ensure compliance with regulatory standards. Provide patient and
family education on disease management, medication administration,
and health maintenance to promote self-care. Document all patient
interactions, care plans, and clinical outcomes accurately and
timely in accordance with organizational policies and regulatory
requirements. Facilitate communication between patients, families,
and healthcare providers to ensure continuity of care and address
any barriers to treatment. Identify and address potential safety
risks in the home setting and implement appropriate interventions.
Participate in quality improvement initiatives and maintain
up-to-date knowledge of best practices in home health nursing.
Skills: The RN Field Case Manager utilizes clinical assessment
skills daily to evaluate patient conditions and develop appropriate
care plans. Strong communication skills are essential for educating
patients and families, as well as coordinating with
multidisciplinary teams to ensure cohesive care delivery.
Organizational and time management skills enable the nurse to
prioritize visits and manage documentation efficiently while
maintaining compliance with regulatory standards. Critical thinking
and problem-solving abilities are applied to address complex
patient needs and adapt care plans in dynamic home environments.
Additionally, proficiency with electronic health records supports
accurate documentation and facilitates seamless information sharing
among care providers. 5-8 hour shifts on orientation, 4x10 hour
