Job Description:
GHR Healthcare is seeking a Nurse Practitioner Home Health for a
per diem / prn job in Lakewood, Ohio. Job Description &
Requirements - Specialty: Home Health - Discipline: Nurse
Practitioner - Duration: Ongoing - Shift: 8 hours, days, flexible -
Employment Type: Per Diem - Employment Type: Locum Tenens
(PRN/Part-Time Nurse Practitioner) - Travel: Daily travel to
patient homes (up to 120 miles/day depending on territory) - Shifts
per Week: 5 (minimum 4 full days/month required; weekday coverage
preferred, weekends a bonus) - Shift Duration: Part-Time (8-hour
field days, flexible start at 7:00 or 8:00 AM) - Estimated Pay
Range: 2925-3450 per week ($105 per-assessment payment model plus
travel stipend) - Assignment Length: Ongoing PRN/Locum coverage •
Accepted NP specialties: AGNP (Adult Geriatric Primary Care), FNP
(Family Nurse Practitioner), ENP (Emergency Nurse Practitioner),
ACNP (Acute Care Nurse Practitioner) • Minimum 1 year of
independent Nurse Practitioner experience (new graduates not
accepted) Nurse Practitioner Responsibilities • Conduct annual
in-home Health Risk Assessments for geriatric patients (1:1 visits,
no follow-up care required) • Perform physical exams, medication
reviews, and evaluate social/environmental factors • Complete basic
labs (e.g., A1C, fit tests – no blood draws) • Document and submit
assessments via provided tablet (same-day submission required) •
Adapt to various home environments and manage logistical
unpredictability (route changes, no-shows, cancellations) •
Maintain high-quality standards and timely documentation •
Communicate effectively with patients and the clinical support team
