Job Description:
Full-Time, 40 Hours/Week Day Shift Onsite Summary: The Home Care
Coordinator assesses the patient's/family's need for respiratory
therapeutic modalities in the home or alternate/extended care
facility. They are responsible for the deliberate organization of
patient care activities pertaining to respiratory modalities
between two or more participants (including the patient and/or
family) involved in a patient's care. In collaboration with the
physician, nursing and care managers, the homecare coordinator
plans, implements, coordinates, and evaluates the education and
plan of care for the family and direct care providers within the
practice setting and across health care settings. Responsibilities:
Provides direct respiratory care to patients and families in
accordance with the scope of practice for respiratory care.
Collaborates with the staff, patients, families, and the community
to assess and meet learning needs and care continuum. Collaborates
with multidisciplinary groups within Akron Children's Hospital such
as PICU, NICU, TCU, Pulmonary, and Palliative Care in regards to
home going needs for complex technology dependent patients. Plans
and implements educational activities to meet identified learning
needs, utilizing adult and child education principles. Develops and
utilizes appropriate evaluation systems to determine effectiveness
of educational activities. Participates in committees, task forces,
meetings and activities to assist in the development of staff.
Provides consultation to influence identified plans of care,
enhance the ability of other professionals and effect change for
improvement of the care plan. Assesses the patient's/family need
for ordered therapy in the home and coordinates home medical
equipment and supplies. Works in conjunction with other disciplines
to coordinate the home going process. Demonstrates the knowledge
and skills necessary to provide care for the physical,
psychological,social, educational, and safety needs of the patients
served regardless of age. Other duties as required. Other
information: Technical Expertise Experience in clinical pediatrics
is required. Critical and analytical reasoning, astute clinical
judgement, strong communication (verbal and written) interpersonal,
organizational, and leaderships skills required. Experience with
healthcare coordination in a managed care environment, homecare or
community agency is preferred. Proficiency in MS Office [Outlook,
Excel, Word] or similar software is required. Epic software or
similar EMR software is preferred. Education and Experience
Education: Graduate from an accredited School of Respiratory Care
is required. Bachelor's degree is required. Certification:
Registered Respiratory Therapist credential. Valid Ohio License as
a Respiratory Care Professional (RCP) is required. BLS, NRP, PALS,
ACLS certification is required. Years of relevant experience: 3 to
5 years is preferred. Years of experience supervising: None. Full
