dynamic and compassionate Project Director to provide high-level
oversight of CCBHC grant compliance, implementation and monitoring
of evidence-based practices, and other required grant activities.
Using a shared governance model, the Project Director convenes and
provides leadership for Centers for Family Development, Inc.’s
integrated holistic services inclusive of behavioral health (mental
health and substance use) and physical health screening and
monitoring. This role focuses on the operational success and
sustainability of the CCBHC initiative. Join us as we transform

Leadership and Oversight Bi-Weekly meetings will engage the
Continuous Quality Improvement Manager, Project Evalutator, Vice
President of Behavioral Health, Medical Director, COO and others as
deemed appropriate to assign task to sustain federal certification
and obtain state demonstration. Ensures fulfillment of contractual,
accreditation and regulatory requirements in the areas of
personnel, operations, claims submission, credentialing,
implementation timeframes, documentation, and all other governing
requirements for the CCBHC certification. Collaborate with the
Medical Director to ensure that integrated care services are
provided holistically for behavioral health recipients that reflect
the inclusion of their physical health needs into their full array
of agency services. The Project Director will receive reports and
environmental scans from the Medical Director on behavioral health
and physical health and the impacts on integration trends and
patterns for the purpose of ensuring CCBHC service array expansion
meets consumers’ holistic needs. Collaborates with the Vice
President of Behavioral Health to align project activities with the
broader organizational behavioral health strategy while remaining
responsible for CCBHC-specific performance, compliance, and
sustainability. Coordinates with the Vice President of Behavioral
Health and Continuous Quality Improvement Manager to align
data-driven improvements with organizational benchmarks.
Participate in evaluation meetings with the Project Evaluator, Vice
President of Behavioral Health, and Continuous Quality Improvement
Manager, and others as deemed appropriate to review progress,
outcomes, and data trends related to CCBHC performance. The Project
Director shall supervise the Practice Manager to ensure that clinic
operations for both children and adults are conducted efficiently
according to integrated needs as a result of primary care screening
and monitoring. This position requires some travel to and from CFDI
clinical offices to manage CCBHC administrative and consumer
relations. Additional travel to trainings maybe necessary. Project
Development and Certification Align and monitor fidelity of the
nine core CCBHC services, as described in the updated Certification
Criteria, Program Area 4, Scope of Services with CFDI’s operations:
Crisis mental health services, including 24-hour mobile crisis
teams, emergency crisis intervention services, and crisis
stabilization. Screening, assessment, and diagnosis, including risk
assessment. Patient-centered treatment planning or similar
processes, including risk assessment and crisis planning.
Outpatient mental health and substance use services. Outpatient
clinic primary care screening and monitoring of key health
indicators and health risks (e.g., BMI, blood pressure, tobacco
use, HIV/Viral Hepatitis). Targeted case management. Psychiatric
rehabilitation services. Peer support, counselor services, and
family supports. Intensive, community-based mental health for
members of the armed forces and veterans. Identify and implement
any operational and infrastructure changes needed to sustain the
certification criteria and improve the quality and effectiveness of
CCBHC services. These changes may include, but are not limited to,
development or update of: Memoranda of Understanding/ Agreement
with Designated Collaborating Organizations (DCOs) and other
partners (i.e., Certification Criteria 3.c). Health Information
Technology (HIT) systems to facilitate care coordination (i.e.,
Certification Criteria 5.b). Electronic health information exchange
systems to improve care transitions (i.e., Certification Criteria
5.b) including necessary data repository updates to sustain
improvements. Processes and procedures for collecting, reporting,
and tracking encounter, outcome, and quality data (i.e.,
Certification Criteria 5.a). CCBHC-wide data-driven continuous
quality improvement (CQI) systems for clinical services and
clinical management (i.e., Certification Criteria 5.b). Develop and
implement a sustainability plan to support delivery of services
including maintaining the necessary positions to render the
services once federal funding ends; and update the sustainability
plan annually to include projected funding models (Michigan
Department of Health and Human Services State CCBHC Demonstration,
Community Mental Health, and sliding scale). Six months prior to
the start of year 4, as specified within the Certification
Criteria, complete a follow-up needs assessment and update project
activities, including staffing plan, training plan, as needed.
Participate in SAMHSA provided CCBHC Technical Assistance Center
activities. CCBHC Technical Assistance will provide guidance to
CCBHC PDI recipients to promote adherence to the CCBHC model,
certification, sustainability, and the implementation of processes
that support access to care and evidence-based practices.

Substance abuse prevention, use, abuse, addiction, and recovery
management; Relapse Prevention Strategies;
HIPAA/Confidentiality/Recipient Rights/Consumerism; Electronic
Medical records; Contractual policies and procedures. Excellent
time-management, interpersonal, oral/written communication, and
organizational skills Excellent data analysis, communication, and
training skills Problem solver and critical thinker mindset
Effective leadership skills as demonstrated by a track record of
successful project management, staff supervision, career growth
and/or external roles in the greater Behavioral Health community.
Demonstrated ability to interact positively in a culturally diverse
environment. Proficient Computer literacy with demonstrated working
knowledge of information technology. Qualifications Minimum
Master’s Degree in Social Work or related field. Experience in
alcohol and other drug treatment/prevention. Knowledge of client
population and treatment strategies. Experience in grant writing
and implementation, administration, data collection, report
writing, and evaluation. Five years recent Community Health
administrative experience and knowledge of co-occurring disorders
and integrated health treatment. Progressive knowledge in the
history and implementation of Certified Community Behavioral Health
Clinic and/or Community Mental Health (preferred)
